RealActivity started from a stubborn problem in academic medicine: nobody could say, with confidence, how physician time was actually spent. The answer was reconstructed from memory, sampled for two weeks, and stretched across a year - then asked to hold up in a federal audit.

We took a different path. Capture activity as it happens, have a clinician confirm it in under thirty seconds, and keep an auditable record with provenance. That record does two things nothing else can: it stands up in a CMS audit, and it gives you a foundation safe enough to run an AI agent on.
